Surgery Day! Jingles welcome :)

Postby Linden16 » Wed May 25, 2016 4:52 pm

Morning! Today's the big day :o I had to travel into a different province (Alberta) in order to have this surgery by one of the only surgeons in North America qualified to do this procedure. I left on the Greyhound bus at 11:50 PM, slept the entire way on the bus and arrived at Banff Mineral Springs Hospital at 8:30 AM. The hospital is a very small, almost boutique-like place. Very quiet, bright and everyone is lovely. My nurse, Ken, is very kind and even though my IV catheter was tricky to place, he has tons of patience. My room is a two-bed ground level room with a gorgeous view of biking/hiking trails. Each bed has their own hilarious TV, and there's free wifi as well. I checked in and was given a "Management of Post Surgical Pain" booklet that was put together by Dr Hiemstra's clinic, Banff Sports Medicine Group. We went over all my paperwork and everything was correct and complete.

I had to gown up and scrub my operative leg (Left) from mid-thigh to the bottom of my foot with a hibitane scrub for 5 minutes, and then mark my leg with a YES.

Now I'm just watching Love It Or List It while I wait. I am going into surgery around 12:30 or so. So a couple hours to kill still.

Dr Hiemstra just came in and had a quick chat. She's such a wonderful individual. So kind and genuine. She has one patient before me, who has also crazy lax patellas, so it will be a couple hours before I go in. She asked me about my right knee that's been giving me trouble and said she needs to do the left one first as she isn't sure exactly how it will do, and only wants to do surgery once on each knee, so the left one will teach her how to deal with it. I have never felt so comfortable with a surgeon before, even though I am nervous for the outcome.

I did the thing!

Surgery took just over 3 hours, and I was in recovery for 1.5. When I woke up, just like last time, there was absolutely no pain control. I felt like my leg was run over by a harrow and then set on fire.

They used hydromorphone, fentanyl, tramacet, naprosyn, percocets and finally Demerol. It took a good 45 minutes to bring fit from a low 10 to a manageable 6.

The procedures were:

1) MPFL (medial patellar femoral ligament) reconstruction (cadaver hamstring graft that is anchored into two shallow drill holes with screws and then pulled through one drill hole in the femur that extends the entire bone and joint out the opposite side. The grafts end up looking like a triangle, and are very tricky to position and get good tension on.

2) LPFL (lateral patellar femoral ligament) reconstruction using the same technique with 2 more donor grafts. The tension and positioning must be absolutely perfect or the tighter graft will win "tug of war" and cause major tracking problems.

3) Osteotomy of a big bone spur that appeared after the first knee surgery in '07. She literally chiseled it off and ground it down so my tibia is smooth again.

So all in all I have 5 large incisions and an additional 4 for the scopes. In addition to my original 2'inch long scar and 4 per cutaneous holes. Holy moly

KathyK, you're too kind. General anesthetic has always gone smoothly for me. It's just pain post op that never works once I'm awake!

Hey Linden16! If you're bored you can always head over to the training forum. It's a little slow over there, I think because people are just so busy these days we don't have a lot of extra time for discussion. So we could use some more input. I remember you from UDBB and I ALWAYS liked your opinions and advice.

Get well soon!

Update: Post-op Day 2

So yesterday and overnight was terrible. Between my facial swelling, laryngitis and the horrific pain, I am miserable. Once my pain got out of control yesterday afternoon, it never got back under control until the ER Doctor approved a 5 mg dose of IM morphine. Surprisingly, the first dose at 11 pm took half the pain away. At least it managed to stop my tears! The next dose at 3 am worked enough for me to sleep until 7 am.

So I'm now staying here until my face swelling goes down, and my knee pain is managed.

I guess the issue is that I had three separate operations on one knee, all needing drills and chisels through bone.

I tell you, there are two kinds of nurses in this world . One is empathetic and proactive, the other Is in it for all the wrong reasons. I've had both kinds in the last 3 days.

Heh guys :)

I started to get very dizzy, lightheaded and nauseous yesterday around noon. My left eye went blurry, and then an hour later my right eye did the same. I felt like adrenaline was constantly pushing through my body, and felt very very weak. The Dr who was covering my case did a full exam and suggested maybe I had Sjogrens a my history of chronic dry eyes &mouth plus the parotid gland swelling all pointed to it. And considering I have Ehlers Danlos the two conditions often coincide.

So after consulting with the anesthesiologist they both decided it was a good idea to send me via ambulance to Canmore hospital for a head CT. That was my first ambulance ride ever and I was a bit nervous as my mother died in an ambulance 2 months ago en route to Kamloops hospital.

CT was normal, thank god. I went back to Banff and again tried to play the "which pain med will douse the fire on my leg but not cause a headache and nausea?" Game.

We still haven't figured that out, but my eyes are less blurry today despite the headache and facial swelling and dizziness that still lingers.

Thanks for all the jingles. You're all wonderful :)

Oh, I forgot to say that yesterday before the other symptoms, the dr thought because of the facial swelling and headache, I should be tested for mumps and mono, and put into a private room as a precaution. Bloodwork came back negative so now I'm in a private room which C is lovely. I feel crappy and don't want to have to deal with someone else's visitors etc.
